Certainly in patients with diabetes, glycemic control is central to their care, but it's really not the full picture when it comes to cardiac risk assessment and management.
We know that medications such as SGLT2 inhibitors have cardiovascular benefits that
are not fully dependent on the glycemic effect. Although these are glucose-lowering drugs, their cardiovascular benefits really
go-- go beyond simple metric of how much do the glucose levels go down. Targeting hemoglobin A-1C alone does not adequately
address cardiovascular risk, as myocardial dysfunction extends well beyond hypoglycemia.
SGLT2 inhibitors reduce preload, lower filling pressures, and attenuate neurohormonal activation independent
of glucose lowering. Select mineralocorticoid receptor antagonists and incretin-based therapies offer
complementary cardiometabolic benefits, supporting a more comprehensive risk-reduction strategy.
