What does it mean to have a medical home?

Having access to care when needed acutely is very important. “Having a medical home” very simply means that you can see a doctor -- your doctor -- when you need to. This access avoids unnecessary time off from work and/or emergency department visits or hospitalization. Data suggest that patients with a primary care physician or other medical home have fewer medical complications and medical costs.

Having a primary care physician who knows you well and is aware of your medical conditions also is very important. Your primary care physician will perform your health maintenance needs that a specialist would not and also help coordinate your specialty needs as appropriate. The term “medical home” is evolving. The newer concept of a true medical home includes much more intensive care coordination, education and preventive services.
