Physician was thorough but it was clear he hadn’t reviewed other provider notes (or even his own previous notes between visits). Whenever I see him I usually have to correct several items he’s already gone over with me in the past. It’s clear he’s very knowledgeable but either chooses not to or doesn’t have time to do a thorough work up based on previous visits and current visit for whatever I see him for on any given appointment. PAs, residents, or fellows write his notes for him and after reviewing his notes it’s clear things are getting lost. He also bills for an hour even if I literally see him for 5 minutes (I know billing is based on total time with the patient and time spent writing reports, reviewing results, etc) but then I see a visit note where the subjective portion is a paragraph and everything else is simply generated by the EHR automatically to fill in the rest (like vitals, reason for visit, etc).
