A biometric screening can help you learn more about your health. You can talk to your doctor about your screening results and ways to improve your health over time. During a screening, several measurements will be collected – such as your height, weight, blood pressure, cholesterol and blood sugar.
Individual screening results will not be shared with your health plan.

- You can speak to your doctor about ways to lower your risks over time.
Key dates
The 2026 screening period is from Jan. 1, 2026 – Nov. 30, 2026; however, if you complete a screening with your doctor anytime between Dec. 1, 2025 – Nov. 30, 2026, these lab results can count toward your 2026 incentive earnings.
You have until Dec. 15, 2026, to submit a Physician Results Form if utilizing this option.
Prepare for your screening
Here is how you can make sure to have a successful screening experience:
- Drink plenty of water.
- Keep taking your medications, as prescribed, the day of your screening.
- Do not eat or drink anything, except water, for 9-12 hours prior to the blood test to obtain accurate results.*
- Arrive five minutes before your scheduled time.
- Allow about 20 minutes for your screening.
*Fasting is not required to participate in the screening; however, to be eligible for the biometric screening lab values incentive, you must fast to ensure an accurate blood glucose measurement.
Incentives
Complete your screening by Nov. 30, 2026 to earn $50. If three out of four biometric screening lab values are in range, you will earn $125. If your blood pressure targets are also in range, you will earn an additional $25.
