As transcatheter aortic valve replacement (TAVR) expands into younger and lower-risk populations, management of severe aortic stenosis is increasingly centered on lifetime valve strategy rather than isolated procedural outcomes.1 Follow-up from low-risk trials has continued to support comparable major clinical outcomes, including mortality and disabling stroke, between TAVR and surgical aortic valve replacement (SAVR).2
At the same time, emerging follow-up data are shifting attention toward durability, coronary access, and future reintervention planning.3-5 Mid-term outcomes from low-risk trials have generally demonstrated reassuring valve performance through five to seven years, while ongoing 10-year follow-up is expected to provide additional insight into structural valve deterioration and lifetime management considerations.
Current decision-making increasingly incorporates anatomical and procedural factors that may influence future treatment options. Coronary ostial height, sinus dimensions, valve frame characteristics, and implantation depth may all affect feasibility of future coronary access or valve-in-valve intervention.5,6 These considerations are particularly relevant for patients with longer anticipated life expectancy.
Heart team-based evaluation remains central to treatment selection. While TAVR continues to offer important advantages related to procedural invasiveness and recovery time, SAVR may remain preferable in selected younger patients or those with complex anatomy, bicuspid valve disease, or anticipated need for future coronary intervention.3,5,7
As low-risk trial follow-up matures, the field is moving toward increasingly individualized treatment selection that balances procedural outcomes with long-term management planning. Earlier referral, comprehensive imaging evaluation, and multidisciplinary discussion continue to play important roles in optimizing patient care pathways.
