It's either broken or was never formed, fully formed. So I think if you take the word system and you say do we have a system? We don't. And so fundamentally, I think the biggest problem that we have in the country is that we don't have a system. A system being pieces that work together well.

It's largely because medicine grew up as cottage industry, in fact until few years ago most physicians worked in practices of under five physicians per practice and they tend to be independent, thinking independent, functioning in small businesses, and so the larger clinics and businesses that formed, often formed around a specific disease, it's a cardiology clinic, or it's an orthopedic clinic, as opposed to saying, when we're taking care of hundreds and hundreds of thousands of individuals, they come with more than one condition and it takes more than one doctor to fix it.
