The primary symptom of schizotypal personality disorder is an extreme avoidance of social relationships. The disorder often begins when people are young adults, causing a great deal of distress for those affected. Schizotypal personality disorder can be distinguished from schizophrenia because psychotic tendencies, often severe and persistent in those with schizophrenia, are usually mild and short-lived.

People with schizotypal personality disorder may experience social anxiety largely due to their lack of ability to talk with others. Sometimes the disorder includes symptoms that appear psychotic in nature, like when people believe they can read others' minds or control others' thoughts. Another primary symptom of schizotypal personality disorder is that people with this condition have odd speaking habits. They may ramble on or switch topics suddenly. They may also act emotionally distant or act in strange ways.
