What is informed consent for local anesthesia?

An informed consent for any procedure should be read carefully and signed prior to the local anesthesia injection. Risks and complications and a detailed description of the surgery or procedure will be included on the consent form. Consent forms serve multiple purposes including safeguarding the patient, physician, hospital staff, and hospital from mistakes and liabilities. Secondary consents for anesthesia may also have a questionnaire with medical history, allergies, pregnancy status, and list of medications.
