A tight IT band can cause Iliotibial band friction syndrome, due to increased friction between the tight IT band and the lateral femoral epicondyle (the bony area on the outside of the knee). IT band tightness can also cause Patellofemoral syndrome, which can cause pain in the knee, usually on the lateral side. It can also cause pain at the hip where the IT band begins.
