Which works better, ultrafiltration or diuretics?

The Ultrafiltration versus IV Diuretics for Patients Hospitalized for Acute Decompensated Congestive Heart Failure (UNLOAD) study examined this question. The trial involved 200 patients, divided into two groups. One group received diuretic treatment; the other, ultrafiltration.

The key findings of the study revealed that:

  • 48 hours into the treatment, the patients in the group receiving ultrafiltration lost 38 percent more weight as a result of fluid removal, and also experienced 28 percent more net fluid loss than those who used diuretics
  • 90 days after being discharged from the hospital, patients who had undergone ultrafiltration required far fewer hospital readmissions for heart failure than those treated with diuretics

The study also showed that the ultrafiltration group had fewer unscheduled visits to the doctor for heart failure and less trips to the emergency room for heart failure.
