Is euthanasia ever accidental?

Euthanasia is never accidental. Euthanasia involves a deliberate act to end life, even if that act is to omit care and to allow the person to die. Euthanasia by definition, whether active or passive, voluntary or involuntary, is an intentional ending of life.
