Nortriptyline HCl should not be taken with MAO inhibitors because the combination of the two drugs can lead to life-threatening risks. Do not drink alcohol while taking nortriptyline HCl.

Other drugs that may interact with nortriptyline HCl include albuterol and other bronchodilators; antidepressants such as bupropion and trazodone; antidepressants that rely on serotonin, such as fluoxetine, aroxetine, and sertraline; blood pressure medications including clonidine and guanethidine; cimetidine; chlorpropamide; drugs to control the heart including flecainide and propafenone; drugs that treat spasms such as dicyclomine; levodopa; tranquilizers such as chlorpromazine and thioridazine; quinidine; reserpine; stimulants including dextroamphetamine; thyroid medication including levothyroxine; and warfarin. You should tell your doctor if you take any of these medications as he or she may choose to alter dosages.

As the effects of grapefruit juice are not fully known on nortriptyline HCl, you should consult your doctor first if you consume grapefruit juice regularly. You should also tell your doctor about all medications, vitamins, minerals, or supplements that you take prior to beginning nortriptyline HCl.
