My name is Nelson Velasco, 71 years old, suffering degenerative changes of vertebrae and spine discs, that lead to a spinal stenosis. causing severe pain that makes my daily activities imposible. Notes recorded by Kenji Muro, MD on 2/9/2022 at 4:17 PM CST F/E x rays show no instability. MRI C shows C4/5 level stenosis with deformation of the spinal cord. This could explain his LUE and finger symptoms . Let's have him RTC to discuss and strategize. Multilevel chronic advanced degenerative changes of the lumbar spine as above. There is severe spinal stenosis at L3-L4, L4-L5, and L5-S1. I had a laminectomy surgical intervention in the length of a year; first the cervical spine, and then the lumbar spine. Today I am totally recovered, having a normal life and enjoying outside activities with my family and friends. Thank you Dr Muro. Nelson Velasco April 28, 2023
