Patient Reviews
Mar 09, 2021
Being young, I was not a "typical" patient, and my back pain completely stole my life from me. Dr. Hazen took me seriously and treated me with the utmost respect and kindness from the moment he walked through the door. He was empathetic, thorough in explaining what was wrong, and it was because of how much he advocated for me that I was able to have my surgery, and thus, be nearly pain free, less than a week after I first saw him. In pre-op, he continued to advocate for me (even when I felt no one in the pre-op team was). It is because of Dr. Hazen's skill, expertise, and care he has for his patients that I finally have my life back. For the first time in months, I can walk, sit, and stand again. I can't reccomend him enough.

Board Certifications
What’s a board certification and why is it important that my provider has one?

A board certification represents a provider’s dedication to ongoing training in one or more specialties, including the completion of intensive exams. While not all specialties have board certifications, if your provider does have one they’ve taken the extra step to master their specialty and to keep up with the latest advancements in their field.
