How might Proscar affect the PSA test for prostate cancer?
Proscar (dutasteride) may make the PSA test better. If you have a PSA that is going up, and we’re not sure if you have cancer or not, you’re going to get sequential PSA tests. If you go on Proscar or Avodart (finasteride) and if the PSA goes up, it’s probably much more likely that you have cancer than...  Full Post

What foods can I eat to prevent prostate cancer?
A diet that is good for your heart is also good for your prostate, which means a Mediterranean or an Asian approach to eating -- lots of fruits and vegetables, easy on the red meats, and eating foods with good fats like olive oil. There is some data that tomatoes, which contain lycopene,...  Full Post

Can drugs be used in place of chemo or surgery to treat kidney cancer?
The medications that are approved for kidney cancer are for those cancers that have already spread, for metastatic cancer. For localized cancer the best cure is through surgery or ablation.

If the tumor is rather large we might use a new class of drugs called VEGF inhibitors, which prevent the...  Full Post

Who is a candidate for chemotherapy for kidney cancer?
I’m reluctant to give chemotherapy over other treatments when kidney cancer has spread but the person feels fine. On the other hand, if a patient has an aggressive cancer and is sick, having pain, and is losing weight, at this point chemotherapy is required and is quite effective. We can...  Full Post

How can I counteract the effects of hormone therapy for prostate cancer?
One area that we’re very interested in is if exercise can slow the progression of prostate cancer, because we know that when men's testosterone levels drop from hormone therapy, they lose muscle mass. They get muscle atrophy, which can lead to them getting weak and other problems. We're...  Full Post

How does the HSP27 protein affect prostate cancer?
HSP27 is heat shock protein 27. A number of laboratories are looking at HSP27, since it appears to be an important protein for things like progression and invasion. This is strictly in the research realm; it’s not being used clinically at this time, but it’s being looked at as a potential...  Full Post

How does consuming soy products affect prostate cancer risk?
We think that soy probably is pretty good for reducing the risk of prostate cancer. We're testing a new genistein drug. There are impressive studies in mice and in subcultures showing that genistein blocks cancer cells from dividing, growing, and spreading. If you believe these studies, you would immediately...  Full Post

When should the various prostate cancer drugs be used?
Provenge is given before chemotherapy. The sequence approved by the Food and Drug Administration is to use abiraterone and MDV3100 after chemotherapy, but we will be conducting clinical trials to test the effectiveness of using these drugs before chemotherapy.  Full Post

Can I do active surveillance if I have more aggressive prostate cancer?
There are some men who may not meet the exact criteria for a trial of active surveillance. But they may say, "You know, I’d really rather not undergo prostatectomy or IGRT (image-guided radiotherapy), even though I don’t meet the exact criteria for active surveillance.” I think that as long as...  Full Post
