There are many components to facial aging, including changes within the skin, the soft tissues underneath the skin, and the bones of the face. It is very important for patients to understand what problems Botox and Dysport effectively treat, which are wrinkles in the skin caused by facial muscle action. If this is the primary concern to the patient, then satisfaction following injectable Botox or Dysport treatment will be high. Botox and Dysport have limitations, however. They cannot provide a significant "facelift" or "eyelid lift" as descent of the facial soft tissues is effectively treated with surgery, not injectable products. Patients with significant drooping of facial soft tissues are not good candidates for Botox or Dysport. Furthermore, not all facial wrinkles are effectively treated with Botox and Dysport and required additional treatments, such as facial resurfacing or Filler injections. Dr. Stuart Linder answered:There are several contraindications for patients for the use of either Botox or dysport. Distant spread may lead to muscle weakness, blurred vision, ptosis of eyelids, dysphagia, urinary incontinence and severe breathing problems. In general, breathing and swallowing problems may be Fatal. This risk is higher in children and adults. Other adverse reactions include detrusor overactivity associated with urinary tract infection, chronic migraine, cervical dystonia, axillary hyperhidrosis, retrobulbar hemoatoma, corneal exposure and ulceration, and spasticity in extremity. Contraindications with Dysport: any hypersensitivity of botulinum toxin, allergies to cow's milk protein, or infected entry site. Should you encounter problems with swallowing or breathing after Dysport injection, call your doctor immediately or receive immediate medical assistance.

Dr. Ellen Marmur answered:Botulinum toxins work wonders on lines and wrinkles, but they do nothing for sun damage or skin texture. For those issues, laser treatments or chemical peels are the best option. Botox has been tested thoroughly since the FDA approved it for cosmetic use in 2002 and has been proven safe. Recently, an Italian medical study done on mice found that botulinum neurotoxins migrated to their brains.1 Doctors read through the literature and were not worried about the ramifications, since any safe medication will be harmful to a small animal if it's overdosed with it. The study was interesting but not clinically relevant to cosmetic use in humans because the dosage is very small and the risk of migration is extremely low. It was certainly taken seriously, and there is a black box label warning, but the judgment remains that Botox and Dysport are safe for cosmetic use. Patients with certain neuromuscular diseases (such as multiple sclerosis) or autoimmune diseases (such as lupus) need clearance from their specialist before considering any injectable.
