Your team starts out with your primary care provider of course,but you may also have an endocrinologist who is a physician who specializes in diabetes management.
Next, you can add the educators in your self-management training class: the RN, the RD and perhaps a social worker or exercise physiologist.
The pharmacist who fills your prescriptions can provide valuable information about your diabetes medications and insulin.
You will see an opthamologist yearly for an exam to screen for and treat any diabetes-related changes in your eyes.
You will also likely add a podiatrist to your list of annual health screenings. He/she will check your feet for diabetes-related changes as well.
Don't forget regular dental check-ups. Elevated blood glucose levels can feed bacteria in the mouth that cause periodontal disease.
Your PCP will add other specialists as are needed. But since you are the person living with diabetes everyday, you are the most important member of this team. If good control and prevention of complications is your goal be sure to keep your health appointments, get your questions answered, and follow their recommendations the best that you can.
