Because the tensor fascia lata (TFL) muscle controls the iliotibial band (ITB), it also influences knee mechanics. A tight ITB will rotate the lower leg outward, which creates stress at the knee joint, especially if the thigh bone is rotated inward. Because the ITB also inserts into the kneecap (patella), it can affect how the kneecap moves when bending and straightening the knee, potentially contributing to anterior knee pain syndrome (AKPS).
