Dr. Sally Beer of Piedmont Heart Institute answered:A transesophageal echocardiogram, otherwise known as a TEE, is a special type of cardiac ultrasound that allows for high-quality images of the heart. It is performed with a special ultrasound transducer that is inserted through the patient's mouth and passed through the esophagus, which is the pipe from the mouth to the stomach. The esophagus is a structure that lies right behind the heart and right in front of the aorta. Because of the proximity of the ultrasound transducer when it is in the esophagus to those structures, very high quality and detailed images can be obtained that are not available with traditional echocardiography that is obtained through the chest wall. A TEE is often performed when there has been inadequate information obtained through a traditional echocardiogram, known as a transthoracic echocardiogram, or when more information about the aorta itself is needed. It is typically performed with some mild intravenous sedation as well as local anesthesia to the back of the throat so that the patient is very comfortable throughout the procedure.

Dr. Sara Mobasseri of Piedmont Heart Institute answered:A transesophageal echocardiogram is an ultrasound of the heart that uses a probe which is advanced down the esophagus of a patient's body to look at the heart’s specific structures in better detail. This procedure is different than a transthoracic echocardiogram, which is done outside the body. In these circumstances patients usually require conscious sedation and the back of the throat is anesthetized with a topical anesthetic. In this manner the patient is quite comfortable when a endoscopic long, thin probe is advanced down the back of the throat. In the esophagus the probe can then obtain better pictures of heart valves, veins and other structures, which would not normally be visible on a transthoracic echocardiogram. These transesophageal echoes are normally ordered in certain instances such as looking for bacteria on heart valves called endocarditis, and evaluating for blood clots in the heart after someone has had a stroke.

The echo transducer, a device that produces high-frequency sound waves, is passed down the esophagus (food pipe) to look closely at the heart valves and chambers to check for abnormalities since the esophagus passes directly behind the heart. The TEE is performed while using a topical spray to numb the throat and heavy sedation to avoid discomfort during the test.
Prior to the procedure it is important to avoid eating for six hours; daily medications are usually taken with sips of water. Since heavy sedation is used for this test, patients typically are restricted from driving for the remainder of the day and should take it easy at home. Patients are required to have a responsible person accompany them home.
Most commonly after the procedure people feel a bit groggy for a few hours and have a sore throat that can be treated with lozenges. There are small risks associated with this test, including reaction to the sedative, or rarely, loss of a tooth, damage to the throat or esophagus or difficulty breathing.
