Dr. Stuart Linder answered:Delayed prosthetic breast reconstruction is usually a 2 stage procedure. The first step includes the mastectomy with placement of the tissue expander. The temporary implant expander will be serially inflated over 8-10 weeks. During the second surgery, under general anesthesia the tissue expander, port and tubing are removed and a permanent silicone gel implant is placed. In patients undergoing radiation to the breast, it often safer to place No prosthetic device or implant due to the High incidence of scar tissue formation to the chest wall. These patients may be reconstructed later with a pedicle or free tissue transfer and possible implant.
