Warnings for bilberry supplements (as well as for bilberry extract and bilberry tea) pertain mostly to high doses or long-term use of products made from bilberry leaves rather than from the fruit. Toxic symptoms include muscle spasms, weight loss and, in rare cases, death. People with diabetes should be careful when taking bilberry leaf products since these can lower blood sugar. Dried bilberries or products made from the fruit seem to be safe.

As with many natural remedies, you should stop taking bilberry supplements two weeks before surgery and for a while afterward. Skip bilberry supplements entirely if you are pregnant or breastfeeding.
