Do I have to sign a consent form to allow for treatment?

Consent forms do need to be signed for treatment especially surgical. Exceptions, may include emergency or life-threatening circumstances or mental incapacity to make reasonable decisions. All of my surgical patients sign informed consents that specific details associated with risks and benefits of the surgical procedures. 

Yes, you will have to sign a consent for any medical treatment. The physician will describe the benefits and risks to the procedure and allow you to ask any questions that you might have prior to signing. The only time that you would not sign a consent for a medical treatment is if it was a life-threatening situation and you were unable to. If your family member is present the doctor would discuss the treatment and obtain consent from your family member.
