It is important for individuals to be actively involved in the management and treatment of the disease. You must "know your numbers." This means knowing your blood glucose levels including hemoglobin A1C, blood pressure readings, microalbumin levels, eGFR, cholesterol, and triglycride levels. To do this, you must monitor and record blood pressure and blood glucose readings between provider visits, make at least annual visits for check-ups, and ask questions about these readings to understand what they mean for you and your diabetes! Then, you and your provider will be able to create a prevention plan specifically tailored for you.
Proper diet and exercise is also important. A dietician is a great asset to enlist on your prevention team also!
