    Johns Hopkins Medicine answered:
The most obvious determinant of post-operative erectile dysfunction is pre-operative potency status. In other words, how easily erections were achieved and maintained prior to surgery. Furthermore, post-operative erectile dysfunction is compounded in some patients by pre-existing risk factors that include:

    - Older age
    - Conditions like cardiovascular disease and diabetes mellitus
    - Lifestyle factors such as cigarette smoking, obesity, and physical inactivity
    - Medications such as anti-hypertensive agents that have anti-erectile effects

If the neurovascular bundle/cavernous nerve are resected or permanently injured, erectile function will be unlikely to recover.
