What information is requested in a health insurance application?

In order to apply for health insurance, you must complete an application. The application will ask for information such as:
- Your name, address and Social Security number;
- Dependents and whether other applicants are covered under Medicare or Medicaid.
- Information about your last policy;
- The date you want coverage to begin and, in some states, whether any applicant has been declined or cancelled from another insurer;
- Your medical history including details of any condition(s) you may have. You might want to request a copy of your medical records from your doctor to make sure you have not left out anything from your medical background.
