You may have to prove your relationship with the child and provide documentation such as a birth certificate. In addition to providing the insurance carrier with the child's full name, date of birth, parents’ names and your relationship to the child, some insurance carriers may request a marriage certificate as well as information about your spouse. You may also want to contact your human resources representative to discuss your specific situation.
