Are all plans subject to the new lifetime rules?

    UnitedHealthcare answered:

    No. The restriction of lifetime limits would not apply to:

    • limited scope vision and dental plans;
    • HSAs/MSAs;
    • retiree plans that have fewer than two active employees.

    The lifetime limits provision also does not apply to a health reimbursement account (HRA) that is integrated with a group health plan.

    These benefits are generally known as excepted benefits under the Health Insurance Portability and Accountability Act (HIPAA).
