    Preventive services must be provided without cost sharing. Preventive services cannot be subject to a lifetime dollar limit. Annual dollar limits are not allowed for plan and policy years beginning on or after January 1, 2014. For years before that date, restricted annual limits will apply.

    Certain procedures may not be fully covered under some benefit plans. Check your health plan regarding your coverage of services.
