What is better for back pain active therapy or passive therapy?

Dr. Mehmet Oz answered:When it comes to healing back pain, research shows that active therapy (you working your own muscles) tends to be more beneficial than passive therapy (like massage and spine manipulation), which is particularly effective for reducing symptoms that may be preventing you from getting moving. Certainly, spine manipulation is better than just rest but way second to active therapy. And be patient, since it takes time to build up surrounding muscles. The data suggest that actively exercising the very core muscles that have pain for 20 minutes three times a week for 10 weeks is the fastest way to be pain free for a long time.

Brian Yee answered:There is a time and place for everything. Typically active therapy is better. However, at times the need for rest or not stressing tissues in acute pain can be better than any movement. There are great manual techniques that can decrease a patient’s back pain. This includes manual therapy, nerve mobilizations, intramuscular manual therapy (formerly known as trigger point dry needling) and other modalities. It is important to incorporate appropriate active therapy interventions including specific exercise prescriptions to improve core and pelvic stability, improving gait mechanics, and integrate functional movement and sport specific movements based on the patient’s pain complaints. There is a time and place for everything.
